When in search of a missing page I often look up the book in Worldcat and ask libraries that hold the edition I want if they will scan the pages for me. I always tell them it is for PG and DP. Some libraries have set up reproduction services as a revenue stream, and those typically have very restricted licences. The following libraries have answered requests for scans. Unless noted otherwise they did so, providing images for useable DP and often PG, mostly hi-res scans. Different libraries within an organization, or even different librarians may have different policies.

The definition of "Republishing" is uncertain. I say that adding a B&W proofing image to DP is not republishing, and I have never had a librarian say no to that. Adding a hi-res illustration to PG clearly violates many of the republishing restriction clauses that I have read.

Some libraries have declined or charge, more often a library that does not want to scan will just ignore the request. I do not have a record of the libraries where I have filled in the web form and not gotten a response.
